Ok this is obviously a parody thread but you KNOW the snowflakes are taking the game in this direction. So if such an absurd circumstance were to be introduced, would the player then need DM consent to heal? After all, they had to consent to taking the damage in the first place so they own it. :P

You need to get consent for each attack and each effect. So if a ghoul attacks three times with the claw/claw/bite, that's express consent for each damage roll and each paralyze effect. After you determine if it would have happened, so that the player can feel properly heroic and dangerous. But wait, there's more. This is supposed to be an RPG. Therefore, as the GM, you need to role play the ghoul asking for and getting express consent for each attack and effect. If no common languages are spoken, then translators must also be found. Could get the whole dungeon involved before it is all over.
It's shortly after the big assembly to resolve this conflict that everything ends in tears. Someone asked the red dragon to mediate, and dragons ain't got time for this nonsense.

While you personally are not at risk, your character certainly is. How much risk is based on the game premise.

Perhaps a DM / GM should run a very lethal fictional scenario, at their gaming table. Then, after the players have lost their first player characters; the ones who stick around to create and play a 2nd character, are the ones you really wanted at your table anyway.

In our group we have on occasion humorously declared 'No, I don't consent to this!' (usually after a failed save or critical hit)

As GM, I do not consent to your requirement of consent to take damage.
