What I used to do was trace part of the outline of one country or state or continent from an atlas, then a piece from another body and so on. I ended up with a natural-looking coastline/river/mountain range that no one recognised.

I like Greyhawk for the reasons mentioned earlier. The setting has all the heroes and villains that were name-dropped in the DMG and other books. It also allows for mid to high-level PCs to make a real difference. In one Greyhawk campaign, our PCs set themselves up as lords of the Bone March after fighting a campaign against the monsters inhabiting the state. This is doable for name-level PCs in Greyhawk because the setting is in a state of chaos, but not all-out world war.

I could see that working, if you do it properly.  The other thing that works is to change scale.  Our minds are remarkable on how they view scale.

Very much so. For one island country (about the size and overall shape of Sicily) I used a piece of the North Carolina coast for the east side, a section of the Falklands for the north coast, and a piece of Greenland for the west. Not only did it look "real" but more importantly, natural.
