Fred Kyle, Smuggler in WEG Star Wars. Tried to blast between two colliding asteroids, screaming "I'm the Nicky Lauda of the Galaxy!!", rolled "8" on 7d6.

Brunatz, 1st level human Ranger: A forester/robber character modelled after Hagrid and Rumcajs (the Czech fairy tale / cartoon character). A widower with multiple children who had flown out to seek adventure, Brunatz was mostly interested in protecting 'his' forest against all threats, including the owners if necessary. Unfortunately, on an adventure on an undead-haunted island, he was lightly wounded by friendly fire, and thereafter charged an evil priestess, whose death touch killed him immediately.

After trying to set fire to the Orc caves, the party is chased out of the cave by said Orcs. Unable to flee from the mass of angry humanoids...

Guybert: 2nd Level Cleric. Took a serious wound from an Orc's mace. He managed to stay conscious, but only long enough to feel the crushing blow of the mace as it turned his skull to powder. (Player rolled a 30 on the d30 critical hit table: Skull smashed, instant death).

R.Bull: 2nd Level Thief. Seeing most of his companions either dead (Guybert), delimbed or knocked out, R.Bull gets a steely look in his eye, bids the Magic User flee, and slowly turns to face the Orc horde, raises his bow and starts picking them off one by one. The numbers were too great though, and as his henchmen fall around him, R.Bull succumbs as the Orcs pile atop him.

The magic user got away to later sneak back and gather up the wounded and find a few fresh faces at the keep.

Solg (2st Level Magic User [GGB- Ovate])- Bled out after being cut down by Merle of the Seven Bloody Ears (Bandit Lair: Hex 807)

Goslin (1st Level WWW Gnome Cleric [Hermit])- Killed by an Ogre (Limestone Cave: Hex 403) [This was an interesting one because the player felt his death was unfair.  I pointed out that he walked right into a monster lair as a level 1 cleric without armor and without at least casting sanctuary on himself first, and had he won initiative he might have run away.]

Uranduil (5rd Level G Wood Elf Fighter [Swordsman])- Slain by a Barbed Devil (Shadowgate: Hex 0203) [If I had ruled that minor artifacts ignored magic resistance this one would have gone completely differently]

Victor (3rd Level Assassin [Waghalter])- Slain by Botwin DuJardin upon their meeting (Ophidian Temple-Hive: Hex 0309) [The player got tired of playing an evil assassin, so he rolled a paladin and announced that his new character killed his old one.]

The DM in question was using an S&W hack as a jousting mini-game.  Father Tobit (first level cleric) rolled poorly, as you can imagine.

Interestingly, my previous PC in the tournament was also killed on the first pass, but I don't remember him as fondly, since he was an evil, brutish lout of a knight.
