The system i outline has no extra bookkeeping. You have hp and a threshold level when you get to the threshold you are injured and can no longer regain hd. Your hp cure at 1 per day until you pass the theshold then you can heal with hd again.
The inability to heal with hd is the injury penalty.

One hp total no additional work to do but it feels like you have physical damage and effects.

A poster on enworld (who I normally disagree with strongly) proposed having to seperate methods to select from without making one default. I think this might work and step on few peoples' toes.

Healing surges, etc....My game went in the opposite way.  In my game, the lower healing spells can only be used once a day on a person.  max healing in a day naturally is 3 points.

D&D is not an "everyone gets a ribbon" game.  If you\'re stupid, your PC will die.  If you\'re an asshole, your PC will die (probably from the other PCs).  If you\'re unlucky, your PC may die.  Point?  PC\'s die.  Get over it and roll up a new one.

I forgot to mention that (random hp for monsters) in my playtest feedback but I did harp on the whole healing mechanic and the hitpoint inflation.

I think you're correct but I see two obvious ways for a middle ground solution and have the pre 4e mechanic and the 4e mechanic as options. Overnight healing tied to either HD dice or character level, you could have CON bonus on top also.

Yup they need both. But making HD and one day heals default, in my opinion, will create more fuss than having them as options. They fit easy enough on top of old school hp. Having an optional tag on them should be no obstacle to people who offered countless workarounds to anyone who didn't like 4E.

I don't see why it has to be difficult.  Have randomized hp and HD healing instead of full healing, and for those that want a more 4e experience, have PCs with max hp (no rolling), full heals after rest, and double monster HP.

Only problem is that there needs to be a default in the core system.  I don't want the base game to be "pick and choose from a dozen options."  I think the game needs to be stingy at first because it's generally better when the DM is generous and allows easier options than when he takes them away.
