I run with a general rule of them, are you trying to either do more damage and or get based some type of armor or defense entirely? typically take a shot at -4.

Are you trying to either disable a limb or cripple them? -2 and they get a saving throw to resist.

Are you trying to trip them? throw sand in their eyes, knock them over, etc? they get a saving throw.

I remember RuneQuest had d100 charts, and there was a 1 in 100 chance you shoot an arrow in the cyclops' eye and take him out with a single shot.  It was different, and I liked that, but you'd need a different chart for each attack type vs each creature type.  If someone could find a way to make it more efficient, it would be a cool way to go.

Rolemaster is like that too.  Critical hits produce crippling injuries with appalling frequency.  I was at a con once where a a character was "attacked" by a falling portcullis trap.  Critical hit.  Through the eye.  Instant death.  Apparently he looked up when he should have dived.  Very first thing that happened in the game, too.
