Are you talking without any sort of modifier? Because technically it's 10+, since with mods you can get up to a 16 (assuming a max roll, +3 stat and +1 from a forward or ongoing).

You mean Apocalypse World stole its mechanics from Better Games. DW came out after AW, and uses the "Powered by the Apocalypse" system.

I think it's more or less an accurate, but incomplete, description of the core mechanic in Powered by the Apocalypse games, barring the correction antiochcow already made.

You're missing something, 2-6 is not complete failure, it is the GM makes a move. This can be anything from the pc getting knocked over to the barbarian horde getting careless with their torches and setting the city alight or something advancing a plot behind the scenes. Mere failure is far too dull.
